Public provider records can change. If you notice a possible issue in a SimplyFind listing, send a concise correction request so we can review the directory record.

A four-step correction process
- 1. Identify the record. Include the provider name, NPI if available, and the SimplyFind page link.
- 2. Describe the concern. Briefly explain what appears inaccurate, incomplete, or out of date.
- 3. Share official context when available. If you have a public official source that helps clarify the record, include its link. Do not send private medical information.
- 4. We review the report. We will review the report against available information and determine whether a directory update is appropriate.
What to email
Email support@simplyfind.co with the provider name, NPI if available, page link, and a concise description of the issue. Do not email private medical information, insurance numbers, account credentials, or other sensitive health information.
Providers should also correct the information through NPPES when the underlying source record is inaccurate. That source update is important because NPPES is the public record on which this directory relies.
